The North York Family Health Team (NYFHT) is composed of a multidisciplinary team of Registered Nurses, Social Workers, Registered Dietitians, Nurse Practitioners, Pharmacists, Case Worker, Child and Youth Worker, Chiropodist, Physician Assistant and Family Physicians. Associated with the Department of Family & Community Medicine at North York General Hospital and the University of Toronto. NYFHT is an academic Family Health Team that provides services to over 75,000 patients.

To provide accessible, patient-focused, and family-centered primary healthcare through an interdisciplinary team committed to transforming health knowledge into best practices.

Board of Directors

NYFHT is governed by a volunteer Board of Directors, responsible to the Ministry of Health of Ontario. The Board consists of physician members from each of our seven Family Health Organizations, representatives from North York General Hospital and Community Volunteers.
