Our Commitment
North York Family Health Team (NYFHT) proudly provides comprehensive and compassionate team-based primary care to its patients and community. We are constantly evolving to meet the needs of our community including how to serve our patients differently, how to support our team members through ambiguity, and how we can partner with others to make a difference collectively. We want to make a meaningful and purposeful impact in the lives of those we serve and the broader health system.
Further to this goal, the North York Family Health Team welcomes support persons to accompany people with disabilities anywhere they are needed. Since we provide confidential services at times, support persons may be required to sign confidentiality agreements to ensure that everyone in the room will respect patient-provider privacy.
Accessibility Plan
Employee Training
All NYFHT team members are trained to communicate and provide the best possible care to patients, including persons with disabilities. Employees must complete accessibility training upon orientation and annual refresher training is documented. Staff offer telephone or virtual care options to patients suitable to their needs, and ensure that patients who are accompanied by service animals or support persons are accommodated.
Service Animals
NYFHT welcomes people with all levels of ability and their service animals.
Assistive Devices
NYFHT ensures that all providers are trained and familiar with various assistive devices that may be
used by patients with disabilities while accessing
our programs and services.
Support Persons
NYFHT welcomes any person anyone to be accompanied by a support person.
Communication
NYFHT will provide communication channels to people with disabilities in ways that take into account their abilities.
Notice of Temporary Disruption
NYFHT will promptly notify patients in the event of a planned or unexpected disruption to services or facilities as soon as possible. This includse the anticipated length of time, and alternative facilities or services, if available.
