DIABETES EDUCATION

The NYFHT Diabetes Education Program is a publicly-funded program available to all Ontarians and supports adults that are newly diagnosed or have been managing Pre- or Type 2 diabetes. We offer education in both individual and group settings and we are committed to a collaborative and patient-centred approach. Areas of focus of counselling include nutrition, physical activity, blood glucose monitoring and medication management.  

How to Access Our Program  

Individuals who are interested in accessing our program may either self-refer or receive a referral from their healthcare provider.

Diabetes Education Program (DEP) Individual Patient Appointment Wait Times


DEP Registered Dietitian: 13 weeks
DEP Registered Dietitian & Registered Nurse: 15 weeks
DEP Registered Nurse: 9 weeks
DEP Team (includes a combination of DEP registered dietitian, registered nurse, and/or pharmacist as needed): 1 week

We kindly remind all patients who have been referred to the NYFHT DEP for one of the above services that they must call reception to begin the
appointment process. Referrals made without a patient call will not be scheduled for an appointment.
