Advance Care Planning Day
This Advance Care Planning Day, take a moment to think about your future health care wishes. Start by reviewing our Advance Care Planning Resources, including an informative video on Power of Attorney (PoA) and Substitute Decision-Making – an important first step in understanding how decisions may be made on your behalf if you’re unable to speak for yourself.
Advance Care Planning Community sessions build on this knowledge, helping you reflect on what matters most, make informed decisions, and communicate your wishes with loved ones and care providers. Dates for our Spring 2026 sessions are being finalized. Please check back soon for updates!
Ontario Abdominal Aortic Aneurysm Screening Program
An abdominal aortic aneurysm (AAA) is a widening or bulging of the main blood vessel in the abdomen. Most people with an AAA do not have symptoms, but if it ruptures, it can be life-threatening.
The Ontario AAA Screening Program is designed to find aneurysms early, when they can be safely monitored or treated if needed. Early detection helps reduce serious complications.
Who Should Be Screened?
- Adults aged 65 to 80 years
- Screening is a one-time simple abdominal ultrasound
If you think you are eligible, speak with your health care provider for more information on screening.
Cervical Cancer Screening Program
Effective March 3, 2025, the Ontario Cervical Cancer Screening Program (OCSP) underwent significant changes, with HPV testing becoming the primary screening method for cervical cancer.

Quality Improvement in Dr. Greiver’s Practice
Since 2009, our family practice within the North York Family Health Team has been actively engaged in quality improvement (QI) initiatives to enhance patient care and services. Our clinical office conducts a QI meeting every two to three months; this includes a family physician, nurses, office administrator, patient partners, front staff members, and allied health professionals such as pharmacists, dietitians, social workers, and QI specialists.
