How to Refer
If you are a health care provider looking to refer one of your patients to our programs, please review the summary and referral options for each of our programs here. There are some programs you can refer directly, in which case a referral form is provided. Some programs, patients can refer themselves. For most of our programs, the referring provider needs to be a member of the North York Family Health Team. A directory of providers is found here.
What is a Family Health Team (FHT)?
Family Health Teams are primary health care organizations that bring together different health care providers to delivery and co-ordinate comprehensive primary care for their community. The team will provide health programs and services to and includes family physicians, nurses, social workers, dietitians, and other professionals (for example, psychologists, respiratory therapists, occupational therapists, chiropodists).
These teams provide primary care services based on local health and community needs, with a focus on health promotion, disease prevention and chronic disease management (including mental health and addictions, diabetes education and management, lung health).
To join a Family Health Team, you need to enroll with a physician who belongs to a physician group affiliated with a Family Health Team. You can find a listing of physicians associated with North York FHT here.
Some physicians may accept patients directly while some require interested patients to seek enrolment through Health Care Connect.
Ontario Health Team
NYFHT is a member of the North York Toronto Health Partners (NYTHP) – Ontario Health Team (OHT). NYTHP is a partnership of health care organizations, family physicians, patients and caregivers who are working together to improve care in North York. With an overall goal to organize and deliver care that is more connected and to make the system of health providers easier to navigate.
Primary Care Network
The North York Toronto Health Partners (NYTHP) is built on the foundation of strong primary care physician leadership as it understands that primary care providers have tremendous insight into the challenges facing our system and, therefore, have unique perspectives on how to address them.
Our NYTHP Primary Care Network (PCN) was established in April 2021 to represent the voice of primary care in North York. The Primary Care Advisory Council (PCAC) is composed of 11 primary care providers and a patient and caregiver health council member to represent the broader primary care network of providers in NYTHP decision-making and service planning. PCAC members sit at numerous OHT committees to provide the primary care voice in OHT planning.

Academic Mandate
NYFHT believes in sharing our expertise and collaborating with others to amplify the impact of team-based primary care.
Our model of team-based primary care, also known as interprofessional primary care, fosters an environment where our team members work together to wrap care around the patient along their health care journey. We have extended our model across our community, and it is now one of the largest in the province. More recently, along with other health care organizations in our community, the North York Toronto Health Partners Ontario Health Team was established. Our shared purpose is to promote health, wellbeing, and positive experiences for all and to create collective impact.
As a leader in team-based primary care, we will amplify our voice to advocate for our community to have the resources needed to advance on our vision. We will augment our breadth of research and publish our results for all to benefit. We will raise outreach to other healthcare providers to train and share our expertise. Our commitment is to empower the next generation of health care providers so, wherever they go, they can mobilize research to policy and practice for the betterment of communities and patients receiving team-based primary care.
