Our Commitment
We are committed to patient privacy and to protecting the confidentiality of the health
information we hold.
Collection of Personal Health Information
We use and disclose your personal health information as necessary, to:
- Provide a continuum of care
- Monitor and evaluate the quality of care and outcomes resulting from our services
- Quality improvement planning for the development and delivery of better services and assess resource utilization
- Fulfill other purposes permitted or required by law
Your Choices
- You or a person acting on your behalf, may access and/or correct your personal health records.
- We take steps to protect your personal health information from theft, loss and unauthorized access, copying, modification, use, disclosure and disposal.
- We conduct audits and complete investigations to monitor and manage our privacy compliance.
- We take steps to ensure that anyone working within the NYFHT protects your privacy and only uses your personal health information for the purposes you have consented to.
Virtual Care
- In some instances, receiving care virtually provides the same or better benefit as an in-person visit. We will offer this option in these instances.
- There are a few platform we use to provide care virtually. These include: Medeo, Ocean, Zoom, amongst others.
- We take every precaution to ensure the platforms we use are safe, secure and trusted.
- Your continued use of these platforms lets us know you are okay with using them.
Your Responsibility
- To find a private area during a secure video appointment.
- Once you receive a secure message, it is your responsibility to keep this message secure – not sharing it with others, logging out of systems, etc
- You can notify your health care provider at any time if you no longer wish to receive care virtually.
- You should not use virtual care for medical emergencies or other time-sensitive matters. If you require a response, it is your responsibility to follow up by phone call.
- Should you require immediate assistance, or if your condition appears serious or rapidly worsens, you should call 9-1-1 or go to a hospital or health care provider immediately.
