What is the North York Family Health Team (NYFHT) Patient Advisory Collaborative (PAC)?

The North York Family Health Team’s (NYFHT) Patient Advisory Collaborative (PAC) is foundational to supporting our commitment to person-centered care. The NYFHT PAC is a group of volunteers who help to ensure the patient, family member, caregiver and community perspectives are embedded in activities to enhance and improve patient experiences. Through partnership and collaboration, the PAC identify opportunities for improvement and are consulted as needed on a variety of activities.

Why is it important?

The NYFHT PAC enables the evolution of patient centered care, inclusivity, and patient engagement by providing valuable insights, feedback, and diverse perspectives.

It gives patients, family members, caregivers, and community members the opportunity to take an active role in how we provide care. It can be a platform for building partnerships, seeking resolution to issues, and improving patient communication.

The NYFHT PAC is an important collaborator in developing, improving, or revising our strategic plan, programs and services, and policies.

What do NYFHT PAC members do?

The NYFHT will share planning activities (i.e., strategic, programming, quality improvement) and updates on quality improvement activities regularly.

NYFHT PAC members:

  • Meet on a regular basis to provide input and feedback regarding the various NYFHT planning and improvement activities
  • Members collaborate with NYFHT team members on specific planning and improvement activities
  • Members share their experiences and perspectives to inform improvement activities
  • Members identify opportunities for improvement/share ideas for improvement
  • Members communicate effectively and are respectful of diverse and differing perspectives
  • Members are enthusiastic about improving the healthcare experience for all members of the community
