Quality Improvement in Dr. Greiver’s Practice

Site Location – 4256 Bathurst St, Suite 306

Since 2009, our family practice within the North York Family Health Team has been actively engaged in quality improvement (QI) initiatives to enhance patient care and services.

Our clinical office conducts a QI meeting every two to three months; this includes a family physician, nurses, office administrator, patient partners, front staff members, and allied health professionals such as pharmacists, dietitians, social workers, and QI specialists.

Led by our North York Family Health Team Registered Nurse, the meetings are well-organized with agendas and minutes; between meetings, we do Plan-Do-Study-Act (PDSA) cycles. We audit and present our results at each meeting.

We have seen measurable improvements in diabetes care, preventive

services, vaccinations, STI screening, and osteoporosis management. We are eager to share our successes with the wider community.
